SERVICE COORDINATOR
We are seeking a Service Coordinator (Case Manager) to support individuals enrolled in New York State’s NHTD and TBI Waiver programs. The Service Coordinator is responsible for person-centered planning, service coordination, documentation, and ongoing participant monitoring, in compliance with NY DOH and RRDC requirements. This role is ideal for someone who is organized, compassionate, and experienced in community-based services.
Key Responsibilities
-Conduct person-centered assessments and service planning
-Develop, maintain, and update Service Plans in accordance with waiver requirements
-Coordinate medical, social, and community-based services
-Monitor participant health and safety
-Maintain ongoing communication with participants, families, providers, and RRDC staff
-Complete required documentation, reports, and case notes timely and accurately
-Participate in team meetings, supervision, and trainings
-Advocate for participant choice, independence, and community integration
Qualifications
Required:
- Bachelor’s degree in Social Work, Human Services, Psychology, or related field
Strong documentation and organizational skills
Ability to work independently in the community
Reliable transportation
Preferred:
- Experience with NHTD and/or TBI Waiver programs
Knowledge of RRDC processes
Case management or service coordination experience
Familiarity with Medicaid-funded services
